Serious Injury Report

During the game, if a player receives a concussion or any other serious injury that involves a trip to the hospital, please notify our Health & Safety Officer.



  • Disoriented/Confused
  • Loss of co-ordination
  • Feeling dazed/stunned
  • Blurred vision
  • Headache
  • Ringing in the ears
  • Sensitivity to light
  • Slurred Speech

Immediate Treatment:

  • Loss of consciousness
  • Repeated vomiting
  • Severe neck pain
  • Seizures (fits)
  • Weakness, tingling, burning in limbs
